Adopted on 30 November 1978. The blue field represents the Pacific Ocean. The stars represent the four federated states: Chuuk, Pohnpei, Kosrae and Yap, arranged like the points of the compass. It is based on the flag of the Trust Territory of the Pacific Islands; a four-star variation of the former trust territory flag was adopted on 30 November 1978.

Four white five-pointed stars, one for each of the federation's constituent states (Chuuk, Pohnpei, Kosrae and Yap), are arranged like the points of a compass on a plain light blue field, a four-star variation of the former Trust Territory of the Pacific Islands flag adopted in 1978.
